My Pregnancy Notes—your digital maternity record

Welcome to My Pregnancy Notes (MPN), an online platform designed to help you manage your maternity care. This secure digital platform replaces traditional paper notes, ensuring easier access, better organisation and enhanced security. MPN allows you to register your pregnancy, access medical records and stay informed throughout your maternity journey.

Important: If you have not already had your initial midwife booking appointment, please self-refer online to refer yourself for pregnancy care with us, and you will be able to register with MPN after you have attended your initial booking appointment.

Important: Please do not change settings within MPN, as this may affect your records. 

How to register

  • Step 1: Visit www.mypregnancynotes.com
  • Step 2: Complete the registration form in English
  • Step 3: Verify your account via email or text message
  • Step 4: Select your hospital booking site (Chelsea and Westminster Hospital or West Middlesex University Hospital)
  • Step 5: Add to your Home Screen for quick access (see below)

Key areas for navigation

You can switch between your notes (default screen when you log on) and clinical record by tapping Clinical Record and My Notes in the top right corner.

Providing access to GPs and healthcare professionals

You can show your notes to your GP or other healthcare professionals when needed. In the future, GPs will be able to request third-party access, but they will need to register an MPN account for this feature.

Your midwife will be notified once this feature becomes available.

Remember: Your maternity notes contain confidential medical information. Always be cautious when sharing access.

How we protect your privacy

Your maternity records are stored securely on the Trust’s database. Access is restricted to you and your healthcare team, and all login attempts are monitored for security purposes. 

  • You will receive a unique login via text message
  • Keep your password secure and do not share it
  • Only approved healthcare professionals can access your records
  • Confidential information is managed under confidential mode

Frequently Asked Questions (FAQs)

1. Can I access My Pregnancy Notes on my phone?
Yes! MPN is mobile-friendly and can be accessed via any smartphone or tablet.

2. What if I forget my password?
Click Forgot Password on the login page and follow the reset instructions.

3. Who can see my pregnancy notes?
Only you and your maternity care team have secure access. You can also share your notes with:

  • Your GP
  • Other healthcare professionals
  • Your partner (upon request)

4. Can I print my pregnancy notes?
Yes, you can download and print your records if needed. 

5. What should I do if I spot an error in my records?
Speak to your midwife at your next appointment—they can update your records for you.
