This process is for Chelsea and Westminster Hospital patients who been recommended to start testosterone for low libido.

Step 1: Initial (first) testosterone prescription
You must be recommended testosterone at your last menopause appointment and advised to request your prescription before completing this form.
You will go on an initial trial for 3–6 months.
Step 2: Blood tests
Once you have trialled testosterone for 3–6 months, if have experienced benefits without any side-effects, you may request ongoing testosterone treatment.
To continue treatment, a recent testosterone blood test result is required. Your latest menopause clinic letter will specify whether you need a blood test at your GP practice or at Chelsea and Westminster Hospital.

Step 3: Submitting your result
Once your blood test results are available, you will by notified via Dr Doctor. Please let us know your results to request a clinical review.
If you have benefited from testosterone treatment without experiencing any side-effects, and your testosterone levels are within the normal range, your clinician will write to your GP to request continuation of your prescription.
