This page provides information for funeral directors arranging collection from Chelsea and Westminster Hospital or West Middlesex University Hospital.
Hospital Release Forms
All transfers from the mortuary require a completed Hospital Release Form signed by the family member who registered the death.

Please ensure all documentation is fully completed prior to collection. The majority of releases require the Hospital Release Form to be signed by the family member or friend arranging the funeral before the patient can be released.
Collection arrangements
Collection is by prior agreement with the mortuary team.
Viewings and transfers are managed during standard operating hours unless agreed otherwise.
If the death has been referred to HM Coroner, additional authorisation may be required before release.
Out-of-hours arrangements
Out-of-hours procedures should be coordinated via the Clinical Site Manager where appropriate.
