Trust shortlisted for Royal College of Physicians' Excellence in Patient Care Awards 2025.

Chelsea and Westminster teams have been shortlisted at the HSJ Excellence in Patient Care Awards and the RCP Awards for innovation in digital cardiac care, staff wellbeing, international support and frailty response.

Pioneering Cardiac Digital Care Through Virtual Wards project shortlisted for an Excellence in Patient Care Award.

This recognition highlights the innovation demonstrated in reshaping care for patients with cardiac conditions. By utilising digital technology, the project has enabled the delivery of safer, more personalised care to thousands of patients, supporting them to stay well at home. 

Improving wellbeing for Internal Medicine Trainees across London shortlisted for 'Developing Workforce – Empowering and Supporting Teams' award.

Team members: Dr Emma, Dr Nadia, Dr Daniel, Dr Beth, Dr Ruth, Dr Alexander, Dr Oliver, and Dr Jesal.

Recognising the rising risk of burnout among Internal Medicine Trainees, especially post-pandemic, the team developed a pioneering fellowship programme to research key wellbeing challenges, provide near-peer support, and deliver targeted interventions. From wellbeing directories and cafes to training sessions and an upcoming Wellbeing Day, their work has already had a positive impact across London and nationally through published research and collaboration.

The North West London International Medical Graduate Programme – Welcoming, Developing, and Helping to Flourish shortlisted for the Excellence in Patient Care Awards.

This achievement recognises the dedication, innovation, and teamwork invested in supporting the Trust's international medical graduate colleagues.  The team has worked tirelessly to develop a programme that not only welcomes IMGs into the NHS but actively helps them to succeed and flourish in their careers. 

The Pre-hospital Ambulance Support Team (PhAST) team on being shortlisted for the Royal College of Physicians' Chief Registrar Project of the Year award.

Team members: Medical Director Sanjay and Chief Registrar.

The PhAST system-wide approach aims to improve care for frailty patients in nursing and care homes. Through close collaboration between the Trust, the London Ambulance Service, West London NHS Trust, Hounslow Primary Care Network, and local care homes, the team successfully reduced unnecessary hospital admissions, improved patient outcomes, and delivered care closer to home. 

Instead of defaulting to hospital admission, which often carries risks for older, frail patients, the project has used a collaborative approach to decide the safest and most appropriate care — often enabling people to be treated in their own care home. During the 11-week pilot, 73% of cases were safely managed outside of hospital, improving patient experience, reducing pressure on A&E, and saving an estimated £45,000 in hospital costs. 

Each shortlist recognises the innovative work occurring across the organisation to help ensure patients receive timely and effective care, while also providing support to staff.
