Patient Passport
Supporting patients with learning disabilities at Chelsea and Westminster
A ‘Patient Passport’ has been produced with the aim of supporting people with learning disabilities who come to Chelsea and Westminster Hospital—copies have been distributed to all wards and departments for use by staff with patients who have learning disabilities.
It gives staff important information about these patients and it also includes useful contacts for community learning disability teams.
This ‘passport’ belongs to the person with learning disabilities and should be returned to them when they are discharged from hospital.
