Patient health status questionnaire

As of Wednesday 23 March 2022, Chelsea and Westminster Hospital NHS Foundation Trust will be sending text messages to all adult patients awaiting surgical procedures, operations and investigations at our hospitals. The text message will contain a link to a survey which will first ask if you still need your surgical procedure, operation or investigations. 

If you answer ‘yes’, you will be asked a series of Pre-Operative Anaesthetic (POA) Questions about your health in preparation for your surgery. Completing these questions now will help us plan any further appointments you will need before your surgery, and on the date of your surgery.

For some patients, completion of the survey may mean that you do not need to attend a hospital appointment before your surgery. You will also be asked if you are available at short notice – please indicate if this is the case so we can contact you if dates and times become available sooner than we anticipate. The answers to this questionnaire remain valid for 6 months. 

Please note that if you have more than one upcoming procedure, you only need to complete the form once. If you inform us that you no longer need surgery, we will inform your GP that you have advised us of this and remove your name from the surgery waiting list.
