North West London Integrated Care Pilot shortlisted for national award

The North West London Integrated Care Pilot (ICP)—in which Chelsea and Westminster Hospital is a key partner—has been shortlisted for a prestigious national award for its work in bringing together the care of people living with diabetes.

The North West London Integrated Care Pilot (ICP)—in which Chelsea and Westminster Hospital is a key partner—has been shortlisted for a prestigious national award for its work in bringing together the care of people living with diabetes.

The ICP is shortlisted in the ‘Diabetes Care’ category of the Care Integration Awards 2012 – winners will be announced at an awards ceremony on 4 July.

These awards are organised by Health Service Journal and Nursing Times magazines to recognise, promote and reward the outstanding efforts of staff throughout the country to achieve more integrated pathways of care to meet the needs of patients with long-term conditions.

Integration between healthcare provided in hospital, community settings and GP surgeries is essential to providing flexible, efficient and responsive services.

The ICP won the ‘Managing Long Term Conditions’ of the Health Service Journal Awards 2011 for its work with not only diabetes patients but also patients aged 75 and over. It is clinically led by GPs, hospital doctors and community care professionals working in partnership.
