What we do

We provide a comprehensive service to manage all aspects of health failure care including:

  • Rapid assessment and diagnosis of patients with suspected heart failure
  • Development of a comprehensive management plan
  • Health education and advice for patients, their families and carers

We aim to ensure that all patients receive evidence-based treatments in accordance and that these therapies are reviewed and adapted as and when necessary to ensure optimum care.

We have established links with primary care providers, and good links with other local hospitals to help promote seamless care and ensure that patients’ needs are dealt with efficiently and according to their wishes.

As someone living with heart failure, you can now access your health records on your computer, smartphone or tablet by signing up to the Care Information Exchange.

The Care Information Exchange patient portal is provided by Patients Know Best, a social enterprise company contracted by the NHS.
