How to make an outpatient referral

Hounslow GPs

Please refer via the Referral Facilitation Service (RFS). See the NHS Hounslow Extranet for the relevant proformas. Forms should be emailed via hounslow.rfs@nhs.net or faxed to 0800 756 7754. Practice enquiries: Tel 05511 434910. We are obliged to send any paper referrals received at the hospital back to the Referral Facilitation Service before booking an appointment.

Richmond GPs

Please refer either via the Richmond Clinical Assessment Service (RCAS), or directly through eReferrals. Where services are unavailable on eReferrals, please email a paper referral to wmu-tr.westmidadmin14@nhs.net

Diagnostic cardiology

Open access walk-in service from 9.30am to 2.00pm for GP referred patients requiring routine electrocardiograms.

GP direct access service for transthoracic echocardiograms, 24 hour ECG holter monitoring and exercise tolerance testing.

We provide either same day or pre clinic diagnostic tests for all the cardiology clinics and the Rapid Access Chest Pain Clinic, offering a one stop service on the day of first appointment.

TIA (Transient Ischemic Attack)

If the patient’s ABCD2 score is greater than 4 or they have had 2 or more TIA attacks within 1 week or they are on anticoagulation treatment, the patient should be sent to A&E and this should be faxed immediately to A&E on 020 8321 2516. More information can be found here.
